Can I create a custom event contact form?

Trying to create a custom contact form and not edit the original contact form.

current form is:

Name Email

Phone Number

Comment

Trying to code one that is:

Name Email

Phone Number

Type of Event

Date Desired

Comment

is this possible? can anyone help?

Hi,

Yes, you can add all those fields to contact form. But you must add the name attribute inside each tag you added.

If it’s a .liquid template and you don’t want to edit the original template use an alternate template

I have the second liquid template created but can’t seem to add more options without creating loads of errors. Tried using those dev documents but they don’t seem to have an open ended option.
