Collect information for subscriptions

Hi, I am creating a site for ordering meals for schools and children. For the school orders, I need customers to be able to input children’s individual name, age, class name and dietary/allergen information (eg vegetarian, gluten free). They are ordering ongoing weekly menus via subscription for (eg) 20 children, each individual meal needs to be labelled so the information needs to be captured somehow. Most subscription apps I have looked at don’t accomodate for these extra fields.
