Contact form not submitting after some custom code has been added?

Hi Folks,

I changed some code on my contact form to make a check box before submitting and for some reason now the contact form has not been working for months. When you fill out the details nothing is sent through. Can anyone point me in the right direction it getting it fixed ASAP?
