i want to create form like this and when user submit then one api will call and i got all the info so that i can store that into metafields

On home page of my shopify website, there is a link - Activate.
When user clicks on this link , the form opens up.
I want to create that form and ask user to fill it.
When user fills details, I want to call api and store all the details entered by user in meta fields.
Due to client confidentiality , I am unable to share the url .Please help.
