Custom form elements on product page doesn't capture info/asset

I have added 2 customised form elements to my product page:

  1. 1 x Text input [liquid] to capture a name for a personalised print I’m selling
  2. UploadKit app to enable a customer to upload a photo.

Both do not show up in the cart preview or cart main page at all.

I have a file/screenshot.

Please help.
