Custom page needed - or can I use an app?

We’re looking to build a custom pick your own page like baked by melissa which allows customers to pick and choose their own custom boxes of cakepops, see here: https://www.bakedbymelissa.com/treats/pick-your-own/pick-your-flavors-25.html

Which app would allow me to do this - or is this a custom webpage that would be needed to get designed?

This is not a Shopify store but you can either have this custom developed into your site or pay for a shopify app
