Customer survey

Hello, we are looking for a customer survey app:

  1. We want to send it one week after delivery of the item.

  2. It should include 5 questions.

  3. It should have an option for stars rating.

  4. Available free plan, if possible

There are many free apps which you can use

also, for survey you can create quizzes on you site and can collect data accordingly also recommended several products in the end

Zigpoll is probably your best bet for this. They have a solid free plan and you can trigger email surveys based on fulfillment status (like X days after delivery). You can easily set up 5 questions and include star ratings.

Kno Commerce is another good one we use on our stores, but their free tier is a bit more limited.

Just a heads up - triggering exactly ‘after delivery’ means your tracking statuses need to sync perfectly with Shopify. If that’s spotty, it’s usually safer to just trigger the email 10-14 days after the order is fulfilled.

On the “one week after delivery” piece, worth knowing that most survey apps trigger off the fulfillment event, not the actual delivery. If your carrier pushes delivery webhooks back to Shopify (Shopify Shipping does this reliably, most others don’t), apps like Zigpoll can read that. If not, you’re usually triggering off fulfillment date plus estimated delivery time, which can be off by several days.

Easiest workaround I’ve seen: Shopify Flow triggers “Order fulfilled” with a wait step of 10-14 days (covers the delivery delay buffer), then hits your survey app via webhook or email. Saves you from having to match carrier status.

Are you selling in regions where delivery times are pretty consistent, or mixed?

  1. You can install a form or survey app (many offer a free plan) to create a custom survey. Some form apps support features like allowing customers to select the related order and auto-filling customer information to reduce friction. You can embed the survey directly on your storefront so customers don’t have to leave your site.
  2. As mentioned by others, you can use the “Order Fulfilled” trigger in Shopify Flow, add a “Wait” action, and then send an email containing a link to your survey form.
  3. Many form apps also provide an embedded interface within Shopify admin to manage submissions. Some can trigger Shopify Flow workflows when a survey is submitted, allowing you to analyze feedback and take follow-up actions—such as sending transactional emails, tagging customers, updating records, etc.

If you’re on Klaviyo you could use a tool like Kinetic to send out forms that your subscribers can fill out inside the email. Usually much better response rates than sending people to a landing page to fill it out. There’s a free plan, so you can just add the form widget to a template in your post purchase flow and see how it performs.
