I have created a form using “Powerful Contact Form Builder”, and now am wanting to have it show up on certain products.
How do I do this?

You can manually add a form to your product description
Or create a template for products that you want to add form then add custom liquid, add form code to that template.

In addition to Dan’s solutions – if you have Custom Liquid Section/Block in your product template, you can paste embed codes there.
