Gather emergency contact at time of purchase

Hello,

How can I gather information from customers at the time of purchase? I found this thread, but don’t know how to add code to my shop. I also don’t want to change the form for all check outs - just one product. Any help you can provide would be great. Thank you!

We sell tickets to summer camp for kids and need to gather the following information at time of purchase:

  • Emergency contact 1: name
  • Emergency contact 1: phone number
  • Emergency contact 1: alternate phone number
  • Emergency contact 2: name
  • Emergency contact 2: phone number
  • Emergency contact 2: alternate phone number
  • names and ages of children being registered
  • Allergies yes/no; details if yes
  • field for parents to enter additional information about children
  • photo permission for marketing and funding purposes: yes/no
