Help with retrieving info from customer which is not in cart page

Hello,

I have a shopify site and I am currently working on this page: https://medifresco.es/pages/dia-de-entrega

the form on the left is an info that I need from the customer.

I am really struggling on how to get that piece of info from the customer, as I try to use to get it.

The problem is that I think attributes can only be used for cart page. How could I get this info from the customer? is there any other name as input that I could use??

Please help, I am really struggling :S

I’m not sure Script Editor is the right tool for this. You might be able to find some way of identifying it in the line item properties, but it would probably be easier to use one of the “product options” apps in the store.
