I want to add a checkbox to my checkout for my customers to select if they give permission for their parcels to be left unattended when delivered. I have attached a screenshot of exactly what I am after.

Only stores on the Shopify Plus enterprise plan can modify the checkout template.
All others need to do this at the /cart step of the process with an advanced theme customization described here:
Or possibly after checkout in some sort of custom process to confirm such a choice possibly in the order-status page.
