How can I add a custom note box to my product in the Dawn Theme?

Hi!

I am using the Dawn Theme for my store www.daughterofanangel.com and want to offer customized orders but was wondering if there is a way to add a custom notes box to one of my products I.E. a box where they can enter a name that they would like on their sweatshirt design.
