Hello, how can I add a area for custom/personalization request like name and a spot to add an image? Using the spotlight theme.

If you need customers to upload their own image, then you would need to use a “Custom Product Options” app (https://apps.shopify.com/search?q=custom%20product%20options). These apps will also allow you to collect text personalization too.
