How can I add a return request page to my Dawn Theme 2.0?

Hello there,

I am currently using the Dawn Theme 2.0. I would like to add a retour request page. I attached a screenshot of a example. How can I make this possible?

Also a extra line with REASON OF RETURN added.

Can you describe more function of retour request page? When submitted, will it send an email to you similar to submit form contact?

@LitCommerce Yes correct. If someone wants to return a specific item I want to have a simple request return template thats sends a email with the customers information to my email.

You can refer to the code to create the contact form and change the field to your liking.
