How can I add custom input fields for customers in my online store?

We sell products that require the customer provide the information. See below

We need fields that allow the customer to put in their “name and rank” prior to ordering.

Any suggestions or an shopify app would be appreciated.

You can use custom field to collect “name and rank”
You have to just place the input fields inside in tag in product template.
