How can I assign different forms to specific pages?

Hello community am having an issue with the shopify forms app:

I have 2 pages that needs forms:

1- build your own brand

2- money making opportunity

i created different forms for each one

when i add the form to one page, it changes on the other!

so form 1 is on both pages or form 2 is on both pages

how can i make each form set on it’s page?

You would need to create a new custom page template:

https://help.shopify.com/en/manual/online-store/themes/theme-structure/templates

And then assign the new template to one of your pages. Then you can go back into the theme editor and edit the form for the new page template, and it won’t change/effect the original page/form.
