How can I create a custom 'Where you find us?' pop-up without slowing my website?

Hello and thanks a lot for your help in advance.I have only one easy question,I want to make on my website www.aiciavem.ro a custom pop up that you only need to select an answear, I wanna ask all customers “Where you find us?” And they have to select an answear.How could I make this without affecting speedscore?

3rd part app always affecting your store speedscore. So create a snippet on your theme may be the best way to do it, and you can save those answer via “checkout attributes” or “cart note”
