How can I create a golf outing registration with product options on my site?

I want to use my site to have a registration signup form, for a golf outing, but I also want to have a product in there that they can select 4 different options and then charge their card once they click signup. Is that possible or how would anyone recommend I do that?

Yes, absolutely. The URL is whitehippogolf.com. I want to have people signup for the outing but they need the option to select different sponsorship packages. Say Gold Sponsor for $1000, then Silver Sponsor for $500, then team for $250, etc..
