Hi! I want to make a page for access code redemption. I want to do this by taking the formatting of the ‘contact us’ page, and changing the ‘phone number’ field to say ‘access code #’. Mostly I want to make a second contact page with slightly different labels while still being able to have the ‘contact us’ page.
yes you can do but make sure this different page same page 2 form doesn’t work

You can follow my steps below to have a second contact page
Step 1: Online Stores > Themes > Customize
Step 2: Click on Pages > Scroll down > Click on create a new template and you should select like this
