How can I create an e-book landing page with a form on my online store?

Hi,

We are currently trying to implement a e-book landing page on our Shopify store. What we need is a basic page which will include a form where people can add their name and e-mail address and once they complete the form and click submit, then a download of the e-book should begin. Also, the e-mail address and name should be stored on a list/segment on our Email service provider which is Klaviyo. We are struggling to find a solution or an app to achieve this. Can you please share your ideas if you were able to solve on your end?

yes i have check your theme if need used details collect on klaviyo nice also you have added details on Shopify admin used default theme contact form
