How can I customize Dawn theme contact us page fields?

How can I make my contact us page look exactly like this? I added this to my contact-form.liquid file but the table lines are all off and don’t look right. 1st photo is currently how it is and I want it to look like the second

Did you attach CSS and the classes required? That would prevent the styling from showing up if the CSS isn’t included with it.
