How can I decode product batch numbers on my website?

Hi, I am looking for an app or other solution, that will allow my clients to enter the batch number of their product (cosmetics) into certain pages on my website to be decoded (each part of the batch number will be related to the origin of each ingredient). For example:

-Client typing in number 11 32 43 45 56

-As a return message he gets:

11 - ingredient x, from location x

32 - ingredient y from location y

43 - ingredient z from location z

and so on.
