How can I embed forms on a collection page using debut theme?

Hi! I was trying to embed a suggestion form into a specific page but I’m not sure where to add it. I’m using powerful form builder. The page where i want it displayed is into a specific collection page. I’m using debut theme and here’s my preview link:
