If you have installed an app to your store which would have edited your theme’s code in any way, then in addition to removing the app by deleting it you would need to remove any edits made to your theme’s code.

Not to worry, as the developers of any such app will be responsible for providing support for their product, including helping to remove any leftover code from their app.

I had a look in the app store for the app shown in your screenshot and it looks like you can find Loadify: Preloader & Speed Ups here in our app store.

You can use the contact details given there to reach out to this app’s developers for assistance.
