I am wanting to implement a contact form onto our website for customers. I want to have questions in places to filter which email address it will be sent to eg Returns questions OR General enquiry? is this something Broadcast-Seventyfour-V1 which our design can do or do we need to add a third party app.

If your theme doesn’t already have built-in conditional logic for forms, you’ll likely need a third-party app to route messages based on user choices (like Returns vs General enquiry). You can custom-code this into Shopify’s native form using Liquid and some backend tweaks, but it’s not super beginner-friendly.
An easier route is to use a form builder with email routing. Elfsight’s Form Builder for Shopify lets you add dropdowns or radio buttons, then set rules so submissions go to different addresses depending on what’s selected. You can also style it to match your store, add file uploads if needed, and connect it to email notifications or Google Sheets.

You can build a custom Contact Us form with dropdowns, radio buttons, file uploads, and other fields, and use conditional logic to show different questions based on what the customer selects.
For a use case like Returns vs. General Enquiry, you can also integrate the form with Shopify Flow to automate what happens after submission—for example, notify different staff members based on the type of request.
Submissions can be managed directly in Shopify admin and associated with the corresponding customer, which can be useful if you receive a lot of customer service requests.
It also has a free plan, which may be enough if your submission volume is relatively low.
