I recommend checking out the Shopify App Store or an available app that will allow you to achieve this. For ease, I’ve gone ahead and made a few suggestions listed below that you can look into:

The free Easify Product Options app is an excellent choice for setting up unlimited product options effortlessly 
. Here’s a quick demonstration on how to use the app to create options similar to the screenshot you provided:
- Create a new option set
- Add an option for customers to select the Keytag material using the Buttons or Dropdown option type.
- Add an option for customers to select the Color using the Color Swatches option type.
- Add an option for customers to select the Size using the Buttons option type.
- Add an option for customers to select the Text Align using the Image Swatches option type.
- Add a field for customers to add their custom text on the Keytag using the Text Area or Text Box option type.
- Assign your custom options to relevant product(s):
That’s it! You’re all set.
