How can I move my size guide to a dedicated page?

Is there a way to add this size guide (which currently lives on my product page), to it’s own size guide page.

For example: https://us.sirthelabel.com/pages/size-guide-1 on it’s own page and they also have it as a pop up: https://us.sirthelabel.com/products/como-twist-dress-paradiso-print#collapseOne

You can do this my creating a new page for sizeguide, and writing few JS to redirect user to the respective page when the link is clicked.

Yes, it is possible to embed size chart similar to the given site. This can unique based on products or same for all using metafields but it needs custom code changes. So please seek help of a developer to help you at reasonable cost.
