How can I resolve multiple item shipping issues?

I am having an issue with my shipping not capturing the number of items.

When my customers purchase the same product multiple items in one transaction my shipping does not recognize it. For example, I had a customer purchase 4 (10lbs) items, and the shipping only recognized and billed for one of the items instead of 4. I have taken losses on some orders and had to cancel multiple. How can I fix this, please?
