How can I restrict a contact form to one page only?

Hi there!

I have a problem. I added a contact form to my contact page https://taneraskin.com/pages/kontakt and now it shows up on ALL my pages, so also on the careers page etc.. How can I fix this??

You need to create another template for your contact page so contact form will not be added to other pages
