How can I set up a login form on a client's website?

I need help for setting this kind of login form on my clients website.

Whether by coding or by using any app I need complete guideline from any expert. I would be very grateful to you if you will help me. Indeed answer as soon as possible.

Before getting into implementation detail, we have few questions.

  1. Are you tagging your existing users with parent or school.

  2. What if user select school and use parent credentials to login.

  3. Have you implemented this at registration page already?

Actually I want login form option here where I pointed, when someone creates account he/she must see the option which I mentioned in above message.

Yes it can be code.

We have to write codes for login and registration pages.

If user choose login as school but use parents credentials he will be logged out.

If you are willing to hire someone, would like to work for you.
