hi! does anyone know how to add this “how did you hear about us?” form on the order confirmation page?

Hi @caseland , what you are asking for requires a 3rd party app, I suggest you check this thread where a similar question got asked and some apps were suggested

Hey @caseland! You can’t add this directly to the order confirmation page unless you’re on Shopify Plus.

On non-Plus plans, the usual solution is to use a post-purchase survey app that shows “How did you hear about us?” on the thank-you page and saves the answer to the order. :blush:

Both replies above are right,on a non-Plus plan you can’t add a form to the order confirmation page yourself; a post-purchase survey app on the thank-you page is the standard route, and Zigpoll has a ready-made template for exactly this question.

One thing worth deciding before you set it up, because it bit me: the dropdown is the easy part. The hard part is that “how did you hear about us?” answers come back one layer too shallow to act on. “Instagram” wins the pie chart but was it a paid ad, a reel from a creator you paid, or a friend’s story? Four different budget decisions in one bucket.

What fixed it for me was asking one follow-up. “Instagram” then “an ad or someone you follow?” then “what about it convinced you?” Suddenly “Instagram” becomes “a before/after reel from creator: the transformation sold me.” That you can act on.

Most survey apps let you add a free-text follow-up question, worth wiring in from day one. Even reading those answers manually on your next 20 orders will tell you more than the dropdown totals will

On a Basic Shopify plan, we cannot add the How did you hear about us?.
For this whether a Shopify plus plan is required or a Shopify app is required.
There are multiple Shopify apps that you can use for the post purchase Survey.
