hi! does anyone know how to add this “how did you hear about us?” form on the order confirmation page?

Hey @caseland ! You can’t add this directly to the order confirmation page unless you’re on Shopify Plus. On non-Plus plans, the usual solution is to use a post-purchase survey app that shows “How did you hear about us?” on the thank-you page and saves the answer to the order. :blush:
