When I export my order I can see a “Notes Attributes” field which I would like to also add to the packing slip. How can I do this please?

I should also add that I am using the salesrep.io app and the code in the form is
{% include ‘salesrep-dropdown’ %}
It shows up on the excel export under Note Attributes

The code works on the cart.liquid page, I am trying to also have those details show on the “packing slip” template.
I have tried all these different options, and I still can not see the order notes
