I want to create something similar to this picture, in terms of the bundle design and layout. Is this done through an app or is it pure code manipulation? If its code, can someone provide some assistance on how to do that?

it is mostly done through apps but some people also used their custom code by using different method like bundle product collection and some use product metafields but only problem with that they can’t manage their inventory with custom code so you should probably go for bundle apps
