How to add a button/banner section in Add To Cart like this? (image inside)

Hi All,

Does anyone know an app or way to add a button to the Cart that’s similar to the image below?

When you add any items to cart, there’s always an option to add a membership. When you click the button, it will automatically add the membership to the cart.

What kind of membership is this, is this product membership or a store membership.

Yes, it can be achieved via custom code in theme.

Thanks for responding! Do you know of any out of the box solutions (ex: apps) that can achieve a similar result?

How big of a project would this be if done custom?
