How to add a personalized card box to a product page?

We’d like to add a space on the product page for adding a card to their order, when they select a card a comments/message box appears for them to add in text for that card.

Here is an example that we like:

Is this an app we can add or does this need to be coded in?

It was added by ‘Gift Wrap Plus’ app, refer https://apps.shopify.com/gift-wrap-plus

You just need to install it, everything will display fine
