I am looking to add a simple contact us section to this price promise page, how would i do this?
I just need to add a small contact us and a few options for them to show evidence of a price found cheaper.

I’m looking to add a form type, with sections.
Such as email, name, price seen, upload file section, that can be submitted to our email.
