Setup Steps:

Go to Admin → Custom Cart → Features → Reward bar and enable it

Set reward basis to Cart total

You should create 3 tiers (for 399, 699, and 899). Add your 3 tiers:
Tier 1: Free shipping at $399

Set threshold to 399 in Cart total field
Tier 2: Free gift at $699

Select your gift products using Search or Browse
