In the meantime, you can explore and test the app with my suggested setup. While the options won’t be wrapped in a popup as you’ve requested, the main options will only appear when the “Personalisation” checkbox is selected, keeping your store looking neat.
Regarding your question, the engraving option will be added as a product with a price and will be shown on the invoice. You can test the app to see how this flows.
