How to add subtext under forms fields?

I recently added forms to my shopify store and they look fine as they are-

However i wanna be able to add text under fields and there is no option to do so.

Here is how my form looks like right now it was made through shopify forms-

My desired layout is like this-

Can someone please help me add text under fields and also the layout, any help is much appreciated.
