How to add "to", "from" and "your message" box beside product

I run a gift box business and would love for people to add their own personal note to the gift they are giving by adding the information to the product page. Does anyone know how to do this? I’d love to avoid paying someone to do this if I can. See attached for example. No drop down necessary
