How to create a custom form without Hulk Apps ads?

I am not so savvy with code and wondering if someone here could help me know how I can create a for that I put together with Hulk Apps. I just don’t like the way they have a huge add displayed at the bottom of the form. I also don’t want to pay $9/month for this. I am using the Narative theme.

Hi and thank you for your reply. I managed to create a form using an app. Its very inexpensive and looks great. The only thing i wish it could do is to give me the option of auto tagging the form with “newwholesale” for the wholesale buyer and they can begin purchasing. Currently i am receiving an email of the form and then i have to go into my admin page to manually tag them.
