How to create a survey that chooses a personalized product

I am looking for an app that can accomplish the effect seen when you press “Get My Formula” on this link. I want to pose a survey to customers that will choose a personalized product and maybe suggest other options within the same light. It would go like this:

  1. Survey

  2. Product suggested based on survey results

  3. Suggestions of products like the one suggested.

Is there any way to do this with Shopify?

We say that you can do this with a “quiz”, but the concept is exactly the same.

Ask questions that help users self-select, and then based on their answers a product is recommended.
