How to include file upload in a custom quote form?

Hi all!

I’m trying to figure out how I can include “ALL” 5 of these selections (1st image) on the quote form (2nd image). As you can see, once I go to the quote form, only the first 4 are included. This is a custom engrave product so I’ll need to see the image uploaded by the customer once they request a quote.

I already contacted the quote app regarding this issue, but haven’t had any solutions yet. So while waiting, I’m wondering if there’s any way around this that I can do?
