How to make custom form for return and exchange without an app

Hi, I want to make a custom form for return and exchange. I want a dropdown list with 2 options Return and second option Exchange. When someone select Return option the form shows fields like name, email, order id and a dropdown list of reasons for return, And when someone select the Exchange option the form shows fields like name, email id, order id and dropdown list of reasons for exchange. Please Help. Thank You in Advance.

You can try to create theme use Google forms

But I want a form like the contact form itself and on my website, if I’ll use google form it will redirect customer to google forms.

  • Create a new page (Contact Template)
  • Include the return text and messaging as you need.
  • Include UI Generator as you need to customize the form.
