I’m trying to add a custom contact form to the Contact Us page. Per the third party’s instructions, I copied the code and added to the content section of the Contact Us page. When I use the page.contact template, the standard Shopify contact form is displayed, not the custom form.

What am I missing here? Should this code be put into the template instead?

I believe you have already got the solution but i would like to share this if it helps someone else like me having this Hulk custom Form display issue. What i missed was embedding the Hulk Form builder in the App Embed area under Theme Customization.

If you’re not using the script tag, please enable the theme app extension from your theme customisation section.
