I want to auto populate fields in a form when an order number is entered

I have an online store that sell online course that are located on an external platform. However I need the purchaser to complete an online form to capture the enrollment details, which can be different from the purchaser. I also need the ability for multiple enrolments of the same course or enrolments of several courses.

See below for the process, which I need help with steps 3-5:

Step 1. Purchaser buys course/s on Shopify

Step 2. Using Zapier, Purchaser is sent an email with a link to a page on my Shopify website to complete enrolment form.

Step 3. On the Enrolment form, the Purchaser provides their order number (sourced from the email)

Step 4. The order number connects to the back end of Shopify and identifies the course/s and SKU details have purchased

Step 5. The enrolment form is auto populated with the course names/s and SKU code/s and creates name and email address fields corresponding to the number of courses purchased.

For example: 2 x Course A and 1 x Course B was purchased. Therefore, the Purchaser sees 2 x name and email address fields for Course A; and 1 x name and email address fields for Course B that need to be populated and submitted.

Step 6. A trigger is set up in Zapier when the enrolment form is submitted, which sends an action to my external platform to create new User/s or updated existing users to be enrolled in course/s identified by the SKU code/s.

Can anyone assist with how I can automate steps 3 and 5 as these are currently being manually completed by the purchaser using a drop-down menu for each enrolment per course, which is not best practice.

It seems that it needs generate a dynamic form based on the order.

Do you use this app generate your form?

Correct. I used POWr.io to create the form, as it integrates with Shopify and Zapier.
