Looking for a form app that has this capability

Hi,

I’m looking for a form app (order form) or a product options app that will let the customer add an additional (duplicate) item and fill out customization options for each item including file upload. So for example, a customer is ordering 10 trophies for a sports team, but wants to customize each trophy with each player’s name. So when the customer is on the trophy product page, the customer can add one, then fill out the fields for name for player #1. Then the customer can ‘add another’ then fill out the fields for name for player #2, and so on. Does anyone know if an app like this exists? Thanks!

Hi @ddmiitw you’d want to research the app store in the product-options category, or for keywords “product personalization” “product customization” “product customizer” etc etc etc

Then probably try to narrow down to any apps that have order-form features, or even CSV upload, etc.

Meanwhile if you don’t need realtime image/3D previews what your looking for can also be done without an app using advanced theme customizations.

Mainly using line-item properties customizations for products in tandem with additional order-form customization.

The image previews can technically be done with just frontend code but it’s just more complicated to setup and manage.

You can use the Easify Product Options app to create text fields where customers can enter player names, using either the Text Box or Text Area option types. There are several ways you could set up your options with the app :hugs: .

Could you provide more details about your product? For example, will customers be selecting different trophy designs and assigning each design to a specific player name, or are they ordering the same trophy in different quantities and adding different names to those trophies?
