The client ordered a kit where they can choose a few items. In the order, I see that they selected 5 items, which is correct. However, the IDs and SKUs of the last item are duplicated.

It can be done from user side, theme or app conflicts, or glitches in the checkout process. Hope you are using the updated theme and apps.
