It seems the app you mentioned above does NOT support multiple checkboxes. It is absolutely important to have two separate checkboxes though.

Are you using the Shopify Plus plan? We can only do this on that plan.

wow, no, considering the price of it it is definitely not an option to upgrade to Shopify Plus with my startup company ! Isn’t there an obtainable solution? If no I would be pretty disappointed and probably need to check for alternatives. I cannot be the only person needing multiple checkboxes for legal reasons.
